Job description

Join BAYADA Home Health Care as a Physical Therapist (PT) to provide high‑quality home health care to clients in Sierra Vista. You will work within a multi‑disciplinary team, develop individualized treatment plans, and use innovative techniques to help patients achieve their goals.

Responsibilities

  • Perform diagnostic tests and measurements such as joint mobility, transfer status, stability, ambulation patterns, strength, endurance, balance, and safety assessments.
  • Develop and implement individualized care plans, including manual therapy, gait training, balance work, and other therapeutic interventions.
  • Apply integumentary protective interventions to prevent and promote wound healing using evidence‑based dressings and topical agents.
  • Continuously assess and revise PT care plans.
  • Educate clients, families, and other caregivers in rehabilitative care and activities to promote health, safety, and independent living.
  • Accurately document observations, interventions, and evaluations using a state‑of‑the‑art tablet computer.

Compensation & Schedule

  • Competitive pay rates with a weekly pay schedule.
  • Flexible scheduling; available for full‑time, part‑time, or per‑diem positions.
  • Relocation assistance up to $30,000 for full‑time hires.

    • Qualifications

  • Current Physical Therapist (PT) license in Arizona.
  • Graduated from a program approved by the American Physical Therapy Association or equivalent board.
  • Strong computer skills; prior electronic medical records (EMR) experience preferred.

    • Location & Working Hours

    Sierra Vista, Arizona. Positions available as full‑time, part‑time, or per‑diem. Relocation assistance provided for full‑time roles.
