Job Description :

Sunbelt Staffing is hiring a Home Health RN in Bluffton, SC. The nurse will deliver skilled in-home care, manage documentation through OASIS, and collaborate closely with the care team. This is a great fit for RNs who value autonomy and patient-focused practice.

Key Responsibilities :

Perform admissions, follow-up visits, and discharge assessments

Document patient care using OASIS and Homecare Homebase (HCHB)

Provide skilled services including wound care, injections, and chronic disease education

Communicate with providers and coordinate interdisciplinary care

Support patients and families with education and resources

Qualifications :

Current RN license in South Carolina

Minimum 2 years of RN experience, with 1 year in home health

OASIS and HCHB experience required

Active BLS certification

Valid drivers license, personal vehicle, and auto insurance

Able to pass background check, drug screen, and meet Joint Commission standards
