Job Description

Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

Sunbelt Staffing is seeking an experienced Home Health RN for a 13-week full-time contract in Shelbyville, KY . In this field-based role, youll deliver compassionate, patient-centered care in patients homes while collaborating with a trusted national home health organization . Responsibilities include performing assessments, creating and implementing care plans, administering medications, and ensuring accurate documentationall aimed at providing the highest standard of home health care .

Contract Details :

Location : Shelbyville, KY, and surrounding areas
Company : Sunbelt Staffing in partnership with a Nationwide Home Health Agency
Contract Length : 13 Weeks (with possible extension upon successful start and completion)
Hours : Full-time, Scheduled 40 hours per week - 32guaranteed hours
Schedule : Monday-Friday, 8 am-5 pm, with occasional weekends and on-call duty (typically one weekend per month on-call)
Productivity : 30-32 points per week, up to 35 patients per week
Compensation : Negotiable, based on experience and territory budget
Mileage Reimbursement : 0.49c visit to visit, 150-250 miles per week expected
Blended Pay Packages : Available for travelers
Benefits : Medical, dental, vision, 401k match after 90 days

Job Responsibilities

Conduct patient assessments using OASIS, ensuring accurate and timely documentation.

Develop and implement personalized care plans in collaboration with physicians and other healthcare professionals.

Provide skilled nursing care, including wound care, medication administration, and monitoring of vital signs.

Educate patients and their families on care plans, medication management, and other health-related topics.

Maintain accurate patient records in Homecare Homebase (HCHB) and ensure compliance with agency standards.

Travel to patient homes within thearea, utilizing reliable transportation.

Participate in case conferences and team meetings to ensure coordinated and comprehensive care.

Requirements

Active Registered Nurse (RN) license in the state of Kentucky.

Minimum of 1 year of home health experience.

Proficiency in OASIS and Homecare Homebase (HCHB).

Current American Heart Association or American Red Cross Basic Life Support (BLS) certification.

Reliable transportation with a valid driver's license.

Ability to meet Joint Commission onboarding standards upon offer.
