About this Position

The Licensed Practical Nurse (LPN) / Phlebotomist / Collaborative Care Manager is responsible for delivering high-quality, patient-centered care through a combination of clinical nursing, phlebotomy, and care coordination services. This role ensures continuity of care, supports behavioral health integration, and enhances patient outcomes through proactive engagement and collaboration with interdisciplinary teams.

Required Qualifications

Licensed Practical Nurse (degree must be conferred on or before agreed upon start date)

Phlebotomist Vocational / Technical Minimum 2 years of clinical nursing and / or phlebotomy experience.

Certifications, Registrations, and / or Licenses :

• LPN license in the State of Oklahoma.
• Valid Driver’s License
