A company is looking for a PT Clinical Navigator.

Key Responsibilities

Conduct clinical intake assessments and provide patient education on treatment options

Monitor patient progress and address barriers to care through personalized strategies

Document patient interactions accurately and collaborate with care teams for seamless experiences

Required Qualifications

Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required

Current, unrestricted PT license in Georgia (Compact License preferred)

Minimum 2 years of relevant experience in physical therapy or MSK-related roles

Willingness to obtain additional licensure in new states as needed

High comfort level with technology and experience using data to optimize clinical workflows
