• JOB DUTIES
• Assess home health pediatric patient for rehabilitative and functional status.
• Regularly re-evaluate the patient's / client's speech and language need and evaluate the outcomes of care on a regular basis.
• Speech Language Pathologist develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care.
• Identifies deficits including but not limited to communication and feeding / swallowing and potential for intervention. Evaluates the home environment.
• Speech Language Pathologist evaluates the need for and recommends communication devices and aides.
• Uses infection control measures that protect both the staff and the patient (OSHA).
• Coordinates services. Informs the physician and other personnel of changes in the patient's / client's condition and needs.
• Speech Language Pathologist demonstrates commitment and professional growth by participating in in-service programs and maintaining or improving competency.
• Supervises, teaches, and provides clinical direction to other speech and language personnel as required or requested.
• Promote Agency philosophy and administrative policies to ensure quality of care.
• Speech Language Pathologist provides effective communication to patients / clients, their family members, team members, and other health care professionals.
• QUALIFICATIONS
• Graduate of an accredited Diploma, with Bachelor's, Master's, or Doctorate degree
• One year of experience as a Speech and Language Pathologist in a pediatric clinical care setting preferred
• Pediatric Home health experience preferred
• Reliable transportation
• Current Texas State license as a Speech Language Pathologist
• Current Driver's License
