🌿 Hospice Registered Nurse (RN) Case Manager Full-Time | Monday–Friday, 8a–5p |Serving Patients Across Leawood & Surrounding Communities

🌼 What You’ll Do

Conduct comprehensive patient assessments and create personalized care plans

Collaborate with the interdisciplinary hospice team to provide high-quality care

Educate and support patients, families, and caregivers on symptom management, pain control, and hospice philosophy

Monitor patient condition and communicate changes to the care team and physician

Offer emotional and spiritual support at end-of-life

Provide care in home settings, assisted living, and skilled nursing facilities

🩺 What You’ll Need

Graduate of an accredited nursing program (Diploma, ADN, or BSN)

Active RN license in the state of Missouri

1+ year of clinical experience (hospice or home health preferred)

Reliable transportation, valid driver’s license, and current auto insurance

Highly preferred to have HOS or SHH experience

Physical ability to sit, stand, bend, lift, and move intermittently (up to 50–100 lbs)
