About the Role
In this vital position, our providers engage in comprehensive health assessments that may take up to one hour, examining a member's health, medical history, medication adherence, and social factors. This unique approach provides deeper insights into their overall well-being compared to traditional office visits. Key responsibilities include :
- Conducting thorough assessments of adult and geriatric patients, which encompasses medical history review, diagnosis, treatment, health education, referrals, and meticulous documentation following Matrix protocols.
- Administering and ordering appropriate point-of-care and laboratory tests as necessary.
- Collaborating with physicians, case managers, social workers, family members, and other healthcare professionals to ensure comprehensive care.
- Partnering effectively with Primary Care Physicians (PCPs) for patient education and follow-ups.
- Delivering services in members' residences, including homes and Skilled Nursing Facilities.

A Master’s Degree in a related field or equivalent experience, with successful NP or PA licensure.Current valid NP / PA licensure in the state of practice, including necessary prescription authority.Board certification in relevant specialties (NCCPA, AANP, AACN, or ANCC).Up-to-date BLS, ACLS, or CPR certification.At least one year of experience as an NP or PA.Able to travel up to 80-90% of the time, including overnight stays and weekend commitments.Possess a valid state driver’s license and proof of automobile insurance.Strong computer skills and experience with medical record software.Excellent verbal and written communication skills.The Matrix Culture
