About the Role

In this vital position, our providers engage in comprehensive health assessments that may take up to one hour, examining a member's health, medical history, medication adherence, and social factors. This unique approach provides deeper insights into their overall well-being compared to traditional office visits. Key responsibilities include :

• Conducting thorough assessments of adult and geriatric patients, which encompasses medical history review, diagnosis, treatment, health education, referrals, and meticulous documentation following Matrix protocols.
• Administering and ordering appropriate point-of-care and laboratory tests as necessary.
• Collaborating with physicians, case managers, social workers, family members, and other healthcare professionals to ensure comprehensive care.
• Partnering effectively with Primary Care Physicians (PCPs) for patient education and follow-ups.
• Delivering services in members' residences, including homes and Skilled Nursing Facilities.
