Hospice RN / RN Case Manager

Salary Range : $48.00 / hour - $53.00 / hour
Work Type : W2
Required Qualifications :
- Active RN license in the state of employment (or eligibility to obtain).
- 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
- A genuine hospice heart : compassionate, empathetic, and patient-centered.
- Comfortable providing care wherever the patient calls “home.”
- Strong communication and critical thinking skills in emotionally sensitive situations.
- Valid driver’s license , auto insurance, and reliable transportation.
- Current CPR certification required.
Preferred Experience (Not Required) :
Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac).Admissions RN, wound care, float pool, or travel nursing.Experience with terminally ill patients or providing serious illness care .Benefits for All Associates (Full-Time, Part-Time & Per Diem) :

Report directly to the Administrator, Executive Director, or Patient Care Manager Senior.Provide direct patient care and support after normal business hours.Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift.Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs.Evaluate patient and family needs to guide clinical decisions and implement the plan of care.Support and collaborate with an interdisciplinary team, including LPNs / LVNs, CNAs, and other support staff.Accurately document clinical visits and communications.Participate in quality assessment and performance improvement programs.Work independently while ensuring continuity of care and coordination with the daytime care team.
