Job Title : Case Manager – Family Structure Restoration (FSR) & Truancy Intervention

Location : Luzerne County, PA

Employer : Abraxas Youth & Family Services

Position Summary :

Abraxas is seeking a dedicated Case Manager to support youth and families in Luzerne County through its Family Structure Restoration (FSR) and Truancy Prevention and Intervention Program. This in-home, community-based role focuses on improving school attendance, family functioning, and long-term youth success through coaching, advocacy, and resource connection.

Daily travel is required for this position.

Pay Rate : Case Manager $24.00 / Hour or Bilingual Case Manager : $25.00 / hour.

Key Responsibilities :

• Assess barriers contributing to truancy and family instability
• Develop and implement individualized action plans
• Collaborate with schools, courts, and agencies to support youth success
• Provide crisis support, parenting education, and behavioral coaching
• Monitor school attendance, routines, and progress toward goals
• Assist families in accessing community resources and services
