Background check

Drug Screen
Collected within thirty (30) days of start; required annually. Date of collection required to be shown on results. Panel must include : Amphetamines, Methadone, Cannabinoids, Oxycodone, Cocaine, Barbiturates, Phencyclidine (PCP), Benzodiazepines, Opiates, Methamphetamine. Negative dilute results are accepted.
Physical
Completed within ninety (90) days of start; required annually thereafter. Must be a complete head-to-toe exam that states CP is fit for duty and no restrictions completed by Physician, NP or PA. General statement without exam details will not be accepted. Physicals completed by a DO will not be accepted.
MMR
Documentation of two (2) MMR vaccines or positive titer results. Titers must be on lab's letterhead or full lab report provided. Decline form accepted for low / equivocal titers, Affiliate form acceptable.
Varicella
Documentation of two (2) vaccines or positive titer result. Titer must be on lab's letterhead or a full lab report provided. Affiliate decline form permitted if unable to provide vaccine records or titer.
Hepatitis B
Documentation of vaccine series, positive surface antibody titer, or Affiliate declination form that is signed after date offer was accepted. Titer must be on lab's letterhead or full lab report provided.
Influenza
Documentation of vaccine for Assignments occurring between 10 / 1-5 / 1. Affiliate decline form permitted. Must follow location's policy for masking during active flu cases.
Tdap
Documentation of vaccine administered within ten (10) years of start date. Affiliate decline form permitted.
COVID-19 vaccine
Proof of vaccination. For those that do not have the vaccine, complete attached declination form. (Client form is requried)
TB
Documentation of two (2) TB skin tests (one (1) TB skin test completed within year of start date and one (1) TB skin test completed within thirty (30) days of start date); TB skin test annually thereafter, or TB QuantiFERON or T-Spot within ninety (90) days of start and annually thereafter. For past positive PPD cases, proof of past positive TB, Chest X-Ray (2 views required) conducted within one (1) year of start.
BBGH Tuberculosis Health Screen Questionnaire
BBGH Client Specific Form; completed within thirty (30) days of start, and updated annually
Respiratory Fit Test
Completed within one (1) year of start. BBGH utilizes the following Respirators : PAPR, 3M 1860, 1860s, 1870, 9205
Color Blindness Test
Completed within one (1) year of start date. Either completed through a clinic or through an on-line testing module with a passing score.
Education verification
Education verification must be for the highest level of education relevant to the postiion working through a third party vendor. Diplomas or transcripts are not accepted. The education institution must match the name of the institution on the BBGH Background Collection form as well as the resume (institution and date graduated)
Specialty Competency Exam
Completed within one (1) year of start for the specialty corresponding to the CP's Assignment. Required annually and 80% or passing score required.
Joint Commission Core I, II, and III modules
Completed within one (1) year of start; required annually. Must include HIPAA training. An 80% passing score is required.
