Licensed Practical Nurse AL MC SNF

LPN _ Assisted Living, Memory Care, Skill Nursing

Duties & Responsibilities

Provide compassionate, high-quality nursing care to assigned patients

Administer and document medications and treatments according to clinical standards

Educate and support patients and their families regarding care plans and health needs

Collaborate with interdisciplinary team members to plan, deliver, and evaluate care

Assist with patient admission, transfer, and discharge processes

Monitor and report changes in patient condition, including vital signs and symptoms

Accurately document care activities and patient responses in the medical record

Maintain a clean, safe, and therapeutic environment for patients

Support daily unit operations and assume charge duties when assigned

Qualifications

Active LPN or RN license Ohio

Minimum 1 year of nursing experience preferred

Strong communication, observation, and critical-thinking skills

Commitment to patient-centered care and teamwork
