Provider MUST be able to do a minimum of two area's listed below :

Mammo / Breast Biopsy together are fine

IR & DX

IR & Fluoro

IR & cardiac

anything with Mammo / Biopsy will be almost an auto approval.

Must be able to do procedures.

Must be willing to help with IR and / or DX call unless they are Mammo / Biopsy

Comment which group they are able to do.

Must have modality sheet uploaded

MUST be enrolled in PCF

PLEASE NOTE : There will be no OT rate for this assignment request

If your bid is accepted, you must submit the credentialing documents 7 days

after acceptance as this is a new start for CHS HBP. Your bid runs the risk of

cancelation if all onboarding documents are not uploaded within the 7-day

timeframe. All credentialing documents are in the "other documents"

tab.
