Essential Functions
Ensure guests receive required medication. Provide health education to guests regarding their diagnosis and / or other wellness topics. Monitors guests’ overall health including BP, weight, and temperature and blood sugar levels as needed. Monitor guests for falls, unsteady gait, wandering, choking, and diet needs.Assists guests with activities of daily living, including, but not limited to showers, toileting, ambulating, meals and snacks, including eating assist and monitoring / recording food intakeCreating and implementing toileting schedules for guests and ensuring CNAs are knowledgeable of each guests’ acuity.Development of individual care plans with approval of Director. Be aware of, follow, and train CNAs on individualized care plans.Assumes the lead and provides support in handling medical emergencies within the day service setting.Attend all care plan meetings and report to co-workers and caregivers any changes as appropriate.Clearly communicate information regarding changes in guest condition to Director.Reports incidents to director and complete paperwork as required.Monitor guest’s health needs and changes in condition. Communicate needs or changes to guest’s caregiver as appropriate.Serves as the liaison between clients, caregivers, physicians, and other professionals regarding medical concerns or issues.Requests Physician orders on all new admissions as well as annually.Follow-up done with Physician offices every twenty-four hours for needed documentationAdministers TB tests and documents results for employees and guests. Tracks tests in “TB Book”.Charts pertinent information in guest progress notes and document monthly on all enrolled guests of Milton Village.Documents any routine or as needed care that is provided during attendance.Completes nursing assessment and appropriate documentation for new enrollments.Records information accurately, thoroughly, and in a timely manner.Knowledgeable of the Milton Village model of care, philosophy, objectives, policies and procedures.Arrives to work and meetings on time and ready to work.Respects confidentiality of guests / families and other employees.Attends educational meetings / conferences for professional growth as required.Sees change as an opportunity and maintains a level of flexibility, which allows for adaptation to new ways of performing.Engages guests through the use of humor, enthusiasm, and positive reinforcement.Performs other duties that may be assigned by director.Job Requirements
