Responsibilities :

Provide professional nursing care including, but not limited to : wound, skin, and ostomy care; admission assessments and evaluations; IV infusions along with care and maintenance of peripheral IVs, PICC lines, CL, ML, and port-a-caths; other skilled nursing care as defined in the plan of care. Assess the patient's clinical needs upon admission and weekly; set goals; recommend appropriate nursing actions to meet those goals. Keep other licensed clinical professionals informed on items related to the patient's care. Monitor and track changes in the client / patient's condition and needs, and keeps the Director of Nursing and any other appropriate personnel informed.
