The Advanced Practice Provider (APP) Specialty serves as a PCP and lead of a specific service or medical specialty. This APP Specialty is a member of the PACE Interdisciplinary Team for planning, collaboration, implementation and evaluation of care plans that meet the objectives, standards and policies of the PACE model of care within the designated service that they oversee.
Essential Responsibilities :
- Conducts thorough and comprehensive physical examinations of participants on an annual and semi-annual basis.
- Evaluates participants in a variety of care settings (homes, ALFs, at the center, etc.) for episodic visits, acute illnesses and follow-up from hospitalizations.
- Makes nursing home visits in accordance with the state nursing home requirements and as clinically indicated.
- Participates in the After Hours On-Call Coverage.
- Provides appropriate health care teaching to patients, their families and caregivers, as well as to other members of the Interdisciplinary Team.
- Adheres to the standards set forth in the Element Care NP / MD Collaborative Practice Guide.
- Completes appropriate, accurate, timely documentation, onsite and offsite.
- Ensures that duties are accomplished in a fiscally responsible manner adhering to established budgetary constraints.
- Ensures that all interactions with clients are conducted in a manner that adheres to the highest standards of care for all Element Care clients.
- Frequent local travel.
- Serves as a PCP at a site as well as the lead in a designated specialty or service highlighted as a need for the population we serve.
- Develops and maintains advanced expertise in the assigned service / specialty (such as with applicable certifications), providing education and support to others.
- Provides consultation and support to other providers and staff regarding specialty specific care plans and best practices.
- Contributes to the training, mentorship and quality improvement initiatives within the service or specialty area.
- Performs other duties as assigned.
- Time dedicated to specialty focus adjusted as needed.
Job Specification :
Master of Science Degree in Nursing (MSN) or Physician Assistant Studies.Current Board Certification as a Nurse Practitioner or Physician Assistant in the Commonwealth of Massachusetts.Massachusetts and DEA Prescription Privileges.Credentialed for the Element Care affiliate Long Term Care Facilities.Current CPR Certification.5-10 years of Nurse Practitioner experience with preference to working with an older adult populationAbility to convey information clearly and succinctly with all employees, so that relevant information is communicated appropriately and in a timely manner.Ability to lead independently and to be able to work well with others.Ability to work within a team concept and to be considerate of co-workers and colleagues.Ability to pay attention to detail, to be a self-starter, and to complete tasks on time.Covid vaccine preferred.EEO Statement
