Home Health Care LPN

The Home Health Care LPN will play a critical role in providing quality medical care and support to individuals in their homes. The primary responsibility will be to perform skilled nursing interventions, educate patients on how to manage their health conditions, administer medications, and collaborate with other healthcare professionals to ensure comprehensive care.

Qualifications :

• Current and valid LPN license in the state of Michigan required
• Valid driver's license and reliable transportation required
• Current BLS for the Healthcare Provider (or completed within 60 days of hire) required
• Proven experience as an LPN, preferably in a home health care setting
• Excellent clinical skills and a solid understanding of nursing principles and practices
• Strong assessment and critical thinking abilities
• Effective communication and interpersonal skills
• Ability to work independently and collaboratively within a multidisciplinary team
• Compassionate, patient-centered approach to care
• Proficient computer skills for documentation and communication purposes
• Flexibility to adapt to varying patient needs and schedules
