Must-Have :
Active monitoring of and amendments to patient visit schedules to maximize productivity and address patient needs.
Actions taken to account for new patients and changes in existing patient schedules, including rescheduling and reshuffling patient loads.
Communication of recommended changes to supervisors as necessary.
Comprehensive assessments of assigned patients, including psychosocial, educational, safety, and risk factor evaluations in accordance with regulations and guidelines.
Development of plans of care to promote optimal functional independence and safety, with patient-specific goals and logical interventions documented in clinical records.
Provision of skilled physical therapy treatment, including therapeutic techniques and patient / caregiver education.
Assessment of patient status and progress during each visit, ensuring quality of care as documented in clinical records.
Coordination of patient care with clinical team members, including communication with physicians, nurses, caregivers, and other rehabilitation services.
Supervision and direction of clinical activities for PTAs, PT students, and home care aides.
Completion of required paperwork to ensure documentation reflects care provided and outcomes achieved.
Accurate execution of charging and billing processes, including necessary documentation for insurance coverage.
Participation in in-service training programs for various stakeholders including staff and community groups.
Maintenance and enhancement of personal knowledge while promoting standards in physical therapy.
Engagement in the marketing and promotion of rehabilitation services through community involvement.
Conducting patient-related health assessments and procedures as necessary.
Timely communication of changes in patient care schedules.
Completion of required time points in accordance with guidelines and regulations.
Collaboration with quality improvement staff to maximize the accuracy of clinical notes and documentation.
Adherence to protocol in conducting visits, including documentation and follow-up processes.
Assessment of patients' needs for additional services, development of plans of care, and ongoing supervision.
Coordination of safe patient discharges and transfers, ensuring completion of appropriate paperwork.
Timely transmission of incident reports to quality improvement specialists as needed.
Participation in regular peer review audits, providing and accepting constructive feedback.
