Position Overview

We are seeking a compassionate and experienced Hospice Chaplain to join our care team, serving patients and families in Kitsap County. As a Hospice Chaplain, you will play a vital role on our interdisciplinary team, helping patients and families explore meaning, purpose, and inner peace during lifes final chapter. This role includes supporting loved ones as they navigate anticipatory grief and loss, ensuring all care honors the beliefs, values, and cultural backgrounds of those we serve.

Primary Responsibilities :

• Provide spiritual and emotional support to hospice patients and families based on their individual beliefs and preferences.
• Conduct spiritual assessments and participate in interdisciplinary care planning.
• Facilitate or coordinate end-of-life rituals, prayers, sacraments, and memorial services.
• Support patients and families through grief, loss, and end-of-life transitions.
• Collaborate with community clergy or faith leaders as needed.
• Offer crisis intervention and spiritual care during emergencies or emotionally difficult times.
• Support staff with spiritual guidance and coping strategies.
• Maintain timely and accurate documentation in compliance with hospice regulations.
• Stay current with best practices in hospice chaplaincy, palliative spiritual care, and relevant professional development.
