Job Title : Registered Nurse (RN) – In-home Assessments Location : Field-based role covering Hudson County, North New Jersey Start Date : Upon completion of training Training Location : Hopewell, NJ (first 2 weeks) Mileage Reimbursement : Yes Position Overview We are seeking a Registered Nurse (RN) for a field-based role conducting in-home assessments for PCA (Personal Care Assistant) and MDC (Major Diagnostic Category) services. This is a remote position in the sense that it does not require reporting to an office, but involves daily travel to member homes in Hudson County, NJ . Key Responsibilities Conduct 3–4 member assessments per day in-home. Evaluate clinical needs using established guidelines to determine appropriate care levels. Coordinate care plans with patients, families, physicians, and other stakeholders. Monitor care delivery and outcomes across service sites. Advocate for members and refer to additional services (MLTSS, SNP) as needed. Document assessments and interactions accurately and thoroughly. Collaborate with multidisciplinary teams via phone and in-person. Participate in quality improvement initiatives and problem-solving processes. Qualifications Active NJ RN license or Compact RN license valid in NJ. Minimum 2 years of clinical experience . Minimum 2 years of health plan (payer) experience . Valid driver’s license and reliable transportation. Strong organizational skills and ability to work independently. Proficient in technology and electronic documentation. Excellent communication and note-taking skills. Note : This role is strictly for Registered Nurses (RNs) . Work Schedule Typical hours : 8 : 00 AM – 5 : 00 PM or 9 : 00 AM – 6 : 00 PM Monday through Friday No specified weekend or holiday requirements
