Read resident logs and other supporting documents as needed upon the start of a shift and record all appropriate entries consistently during and before the conclusion of the shift. Engage patients in social conversation, provide supportive listening, and encourage healthy activities. Establish and maintain a positive working team environment with all registered nurses, providers, and residential facility staff and treat everyone with dignity and respect. Participate fully in team meetings, training, and one-to-one supervision. Provide feedback to oncoming staff regarding resident's interactions, activities, and behaviors at the end of your shift. Report suspected changes in a resident condition and / or potential crisis situations to the charge nurse or floor lead. Remain alert and provide direct line-of-sight monitoring of residents as needed. Administer prescribed medications, follows physician's orders for all medications, and documents on medical charts and count sheets according to established procedures. Work closely with the resident's healthcare team (e.g. nurses, physicians, and counselors) to ensure their overall health is meeting their goals. Other duties as assigned.

Abilities :

• Ability to communicate effectively in both written and oral formats, including the ability to complete objective accurate documentation in a timely, consistent manner.
• Ability to interact with individuals of various backgrounds, education levels, and socio-economic status.
• Must have basic computer skills including the ability to document in an electronic medical record, use electronic time tracking, and the ability to use Google Drive and email.
• Ability to maintain confidentiality, comprehend laws, regulations, and policies, and the ability to follow such laws, regulations, and policies and report when they are being violated.
