The Homestead at Montrose is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.
Hourly Pay : $16.50 - $23.54 per hour based on experience, C.N.A. and QMAP status ! Night shift differential offered at $1.00 per hour.
Schedule : PRN, may be asked to work both day and night shifts; 12-hour shifts

Employee Assistance Program (EAP)Wellness programMinistry ProgramThe primary role of the HHA / C.N.A. / Caregiver is to provide your assigned clients with routine daily nursing care in accordance with established agency nursing care procedures as directed by your supervisors. The Home Health Aide reports any noted changes in the client as they are noted.
QUALIFICATIONS :
QMAP certified (required only in applicable jurisdictions under state [CO] licensing requirements).Be eighteen (18) years of age.Experience in a Home Health Agency or health care facility strongly preferred.Have successfully completed a 75-hour training course and competency evaluation; orHave successfully completed a competency evaluation approved by the Commissioner; orHave successfully completed training in another jurisdiction equivalent to that required in 1 (a); orHave successfully completed a training program or competency evaluation program which satisfies the requirements of Medicare for Home Health Aides as provided by the Code of Federal Regulations, Title 42, Section 484.36.Have successfully passed a competency evaluation administered by an approved training entity on or after July 1, 2017, and whose name appears on the Department’s list of individuals who have passed the requisite competency evaluation.Have satisfied the following requirements :
Meet the requirements of Title XVIII of the Social Security Act for Nursing Assistants in nursing facilities certified for participation in the Medicare program or have successfully completed a Nursing Assistant training program approved by the State of Minnesota; andHave had a least twenty-four (24) hours of supervised training or experience performing Home Health Aide tasks in a home setting under the supervision of a RN or complete the supervised practical training or experience within one-month after beginning work performing Home Health Aide tasks; orPrior to June 1, 1993, had completed an approved Home Health Aide training program.Required Skills :

1. Home Health Functions
Assists clients with personal care activities, including but not limited to :
Bathing – full or partial;Shampoo / hair care;Skin care / nail care, including foot soaks;Oral hygiene / denture care;Shaving;Dressing.Follows all shift routines, policy and procedures regarding accurate performance of vital signs, weights, etc.Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.
Range of motion exercisesAssistance in ambulation or exercisesAssists with ambulation, transfers and exercise.Assist with body positioning or transfers of clients who are not ambulatory.Perform household services essential to health care at home, including, but not limited to :
Meal preparation / feeding;Laundry;Light housekeeping.Assist with bowel and bladder control, devices and training programs.Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines and reporting unsafe situations to the Case Manager.Respond and attend to client requests promptly.Maintain proper hand washing techniques.Maintain a safe client environment.Maintain client confidentiality; treat clients and families with respect.Understand, accept and respond to emotional needs of each client.Reports promptly to the nurse any changes in the client condition.Reports any indication of client illness to the nurse.Provides any information on changes in skin conditions to the nurse.Observe and report any noted changes in mental status to the nurse.Report to charge nurse all incidents, accidents that occur involving clients, and visitors.Follow standard precautions and works within OSHA guidelines.Treats and ensures that clients are treated with respect and dignity.Immediately reports any suspected abuse or neglect per agency policy.Remains aware and takes special precautions to ensure a clean safe environment for clients and staff.Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.Accept and fulfill assignments with the Agency; exercise judgment in accepting assignments.Perform other related duties and responsibilities as deemed necessary.2. Documentation
Accurate documentation of cares given and pertinent observations.3. Personnel Functions
Attend designated home health meetings as scheduled.Develop and maintain a good working relationship with inter-departmental personnel.Attends educational offerings and obtains at least twelve (12) hours of continuing education a year per regulation.4. Committee Functions
Serve on, participate in and attend various committees and meetings of the agency to enhance interdisciplinary process.5. Performance of Routine Procedures
A Home Health Aid may perform delegated medical or nursing and assigned therapy procedures if :
Prior to performing the procedures, the Home Health Aide is instructed by a RN or Therapist in the proper methods to perform the procedures with respect to each client.A RN or Therapist specifies, in writing, specific instructions for performing the procedures for each client.Prior to performing the procedures, the Home Health Aide demonstrates to a RN or Therapist the Home Health Aide’s ability to competently follow the procedures.The procedures for each client are documented in the clinical record.At The Homestead at Montrose , our approach to care grows out of respect for the human spirit and the belief that the right to dignity doesn’t diminish with age.
