Apex Community Health Center is a community-based healthcare organization dedicated to providing accessible, high-quality, and compassionate care to all. We are expanding our team and seeking a detail-oriented, patient-focused Referral Coordinator / Patient Care Navigator to help support our growing patient population.

Position Summary

The Referral Coordinator / Patient Care Navigator is responsible for coordinating patient referrals, ensuring continuity of care, and assisting patients in accessing specialty services and community resources. The ideal candidate is highly organized, compassionate, and able to communicate effectively with patients, providers, and external partners.

Key Responsibilities

Coordinate and process patient referrals to specialty care providers, diagnostic testing, and community resources

Track referral status and follow up with patients and specialists to ensure completion of care

Assist patients in navigating insurance requirements and obtaining necessary authorizations

Maintain accurate records and documentation in the electronic health record (EHR)

Collaborate with providers, medical assistants, and behavioral health staff to support patient care plans

Provide patients with information, education, and support to overcome barriers to care

Ensure compliance with all regulatory and organizational policies

Qualifications

High school diploma or equivalent required; Associate’s or Bachelor’s degree preferred

Previous experience in healthcare (referrals, care coordination, case management, or patient navigation) strongly preferred

Knowledge of insurance authorization processes and medical terminology is a plus

Strong organizational and communication skills

Ability to multitask in a fast-paced environment

Bilingual (English / Spanish or English / Armenian) is a plus
