About The Position

Home Health Nurse Job Description

The  Home Health Nurse  is a licensed professional responsible for delivering skilled nursing care to patients in their homes. This role involves providing specialized nursing services in coordination with a multidisciplinary healthcare team, following physician orders, and under the direction of the Clinical Supervisor.

Key Responsibilities :

Delivering  skilled nursing care  and conducting  comprehensive patient assessments

Providing  education, supervision, and counseling  to patients and caregivers

Collaborating with an  interdisciplinary healthcare team

Utilizing  critical thinking and problem-solving skills  to optimize patient outcomes

Monitoring patient progress to ensure  goal achievement

Completing accurate  OASIS documentation

Administering physician-ordered treatments, including :

Wound care (dressings, vacs)

IV therapy and phlebotomy

PleurX drain management

Fingerstick coagulation checks

Tracheostomy and PEG tube care

Ostomy management

Telehealth monitoring

Job Requirements

Qualifications & Requirements :

Minimum 1 year  of recent nursing experience in a clinical setting (home health experience preferred)

Completion of  Point of Care Testing training  (provided during orientation)

Core Competencies & Skills :

Clinical Experience :  Background in  hospital or acute care  settings

Communication :  Strong  verbal and written  communication skills

Confidentiality :  Ability to handle sensitive patient and team information securely

Crisis Management :  Capacity to remain composed in high-pressure situations

Customer Service :  Professional and compassionate interaction with patients, families, and team members

Technical Proficiency :  Experience with  Electronic Health Records (EHR)

Licensing & Compliance :

Active nursing license

Valid driver’s license with  no more than 3 moving violations

Proof of  current auto insurance
