• Offering Pay-Per Visit Rates - $55.00 Pay-per-Point - $37.00 Base Rate
• Clinical home health / OASIS data assessment tool experience a plus
• Benefit eligibility upon start date - no waiting period

Overview

The Registered Nurse conducts one-on-one patient care and patient / family teaching in the home setting. The RN also performs patient assessments and collaborates with the care team to develop and implement a plan of care. Additional responsibilities include regular communications with our clinical management team and timely and accurate documentation.
