Join our team as a Travel Home Health Physical Therapist (PT) in Sacramento, California. In this role, you will develop and implement individualized treatment plans for patients in their homes, evaluate patient progress, modify care plans as needed, and educate both patients and their families about conditions and the rehabilitation process. You will also communicate with other healthcare team members to coordinate care and document patient information, treatment plans, and progress in a timely manner. Two years of experience in home health physical therapy is required.
