Reliant at Home is seeking a Full-Time Home Health RN Admissions Nurse in the Reliant at Home Fort Worth coverage area. This opportunity has an expectation of 15 Admissions / Week, Monday through Friday, and includes a very competitive base salary, sign-on bonus, and over-productivity bonus. Must have HCHB experience and strong documentation skills.

Responsibilities

• Primary functions are to perform Home Health admissions in the patient home setting.
• Perform Home Health evaluation and assessment per OASIS E data set.
• Develop and document the plan of care in correlation with the assessment.
• Provide skilled nursing care as ordered by physician.
• Timely submission of all required documentation.
• Communicate with supervisor, interdisciplinary team, patients / families, and physician in the coordination of care process.
• Demonstrate commitment, professional growth, and competency.
• Meet productivity standards.
