As a Case Manager specializing in mental health and addiction medicine, you will serve as a critical bridge between clients and resources, ensuring effective access to support services that address their recovery and broader needs. Responsibilities include coordinating treatment plans, maintaining comprehensive documentation, and assessing client progress to adjust strategies accordingly. You will work alongside clinical teams to advocate for clients' welfare, facilitate referrals, and provide emotional support during their recovery journey. The role demands exceptional interpersonal skills, empathy, and a dedication to empowering individuals to achieve sustainable sobriety.
Job Duties
Coordinate and implement individualized case management plans that support clients treatment goals and long-term recoveryServe as a liaison between clients and external service providers, including housing programs, employment assistance, legal support, and outpatient careConduct biopsychosocial assessments to identify client needs, strengths, barriers, and available resourcesFacilitate referrals and ensure follow-through for medical, psychiatric, legal, financial, and social servicesProvide emotional support and advocacy for clients throughout their treatment experience, fostering empowerment and self-efficacyCollaborate with therapists, nurses, peer supporters, and clinical leadership to align case management with therapeutic and medical careMaintain accurate and timely documentation of all case management activities, client interactions, and progress updates in accordance with organizational and regulatory standardsMonitor client progress and participation, revising case plans as needed in response to changing needs or treatment outcomesAssist clients in preparing for discharge, including coordination of aftercare plans, outpatient treatment enrollment, and securing housing or transportationSupport crisis intervention efforts by connecting clients to appropriate emergency services or higher levels of care when neededParticipate in interdisciplinary team meetings and contribute to treatment planning discussionsStay informed of community resources, recovery programs, and benefit eligibility requirements relevant to the client populationUphold ethical standards, maintain client confidentiality, and comply with Ohio regulatory guidelines like OhioMHAS and organizational policiesLicense Requirements / Qualifications :
Bachelors or Associate's Degree1+ years of experience in a detox or residential addiction treatment setting
