Job description

The Community Services Case Manager assists individuals in need of long term support services with making informed choices about setting, services, and financial resources that best meet their identified needs.

Responsibilities :

  • Provide the following services to consumers :

unbiased information about long term support services

  • information about resources to pay for services
  • a person-centered approach that ensures the consumer's goals and preferences are
  • honored
  • options counseling in a timely and appropriate manner based on the consumer's
  • situation and / or disability
  • identification of next steps
  • assistance in facilitating referrals and resources as needed
  • 30 day follow up contact from the Community Services Case Manager
  • Coordinate activities with other case managers stationed at ADRC of Central Mass member agencies under the direction of the ADRC Coordinator.
  • Coordinate activities with other trained agency staff providing options counseling, including but not limited to : l&R Workers, Care Managers, Skills Trainers and Nurses.
  • Document activities regarding counseling sessions, progress notes, and correspondence in a format prescribed by EOEA, MRC and the ADRC of Central Mass.
  • Assist with data collection for the purpose of reporting Community Services Case Manager activity.
  • Complete all documentation required for measuring quality and outcomes.
  • Participate in activities to assist and promote ADRC of Central Mass in gaining visibility and trust with Community agencies, providers and the public.
  • Follow the principles of consumer directed care.
  • Attend statewide and regional ADRC Meetings.
  • Essential Functions / Qualifications :

  • Bachelor's Degree in a human services area.
  • Knowledge of the long term care and social service delivery systems, community resources and the local service systems for elders and persons with disabilities.
  • Valid Driver's License; successfully passing Driving Record Check and meeting minimum insurance requirements.
  • Communication Skills with consumers, physicians, family members, providers (telephone, email or in person, as needed).
  • Communication skills with funders, state and federal agencies, ASAPs.
  • Physical Demands :

  • Must be able to sit for periods of time at computer or while driving.
  • Ability to traverse homes, yards and stairways.
  • Must be able to carry files of up to 5 pounds and laptop.
  • Frequent Standing and walking.
