This position is situated at 620 Sand Hill Road, Palo Alto CA 94304.

Responsibilities :

As a Licensed Practical Nurse (LPN), you will provide care and services to residents by identifying their needs and implementing prescribed interventions. You will also evaluate outcomes and conditions. This role involves performing personal care for residents as necessary, while effectively communicating and collaborating with other members of the care team to resolve resident care challenges and enhance care delivery. Additionally, you will participate in quality assessment and performance improvement initiatives.

Schedule : This position is for the SUNDAY-THURSDAY PM SHIFT (3PM-11 : 30PM).
