Immediate need for a talented Care Manager(LCSW / LMHC). This is a 04 months contract opportunity with long-term potential and is located in Flagler County primarily,   Alachua, Citrus, Lake, and Marion

Pay Range : $38 - $40 / hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities :

• Remote – Making face to face visits at members home or other agreed upon public locale. Flagler County primarily,   Alachua, Citrus, Lake, and Marion
• Position is autonomous with Care Manager to member face to face visits
• COORDINATING CARE FOR SPECIAL NEEDS FAMILY ON THE CMS PLAN
• Set tasking of assigned caseload Max 200 members
• Virtual with a couple of in person meetings per year
• Remote / Autonomous high tech work environment with field visits and telephone contact of members
