You'll partner with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy.\n\nIf you're a compassionate Registered Nurse who values meaningful patient connections and autonomy in your practice, we'd love to meet you.\n\nRegistered Nurse Case Manager Requirements & Responsibilities : \n

• Associates, Bachelors, or Masters of an accredited nursing program (RN)\n
• 1+ years of recent acute care experience required; hospice / home health experience preferred\n
• Complete initial and ongoing comprehensive assessments to determine hospice needs\n
• Develop individualized care plans with the patient and family, updating as needs evolve\n
• Administer medications and treatments as prescribed\n
• Educate patients and caregivers on care techniques, medications, diet, and symptom management\n
• Rotate on-call duties and ensure seamless care transitions\n
• Must have reliable transportation, valid driver's license, and proof of auto insurance\n\nBenefits & Schedule : \n
• Schedule : 8 : 00am to 5 : 00pm, Full-time position with on-call expectations\n

• Paid time off and paid holidays\n\nThe starting salary range for this role is $73,000-$99,000 annual base salary. This range is a good faith estimate; we may pay more or less than the posted range, and this range may be modified in the future. Actual compensation packages take into account a wide range of factors that are unique to each candidate, including but not limited to geographic location; skill sets; relevant education and certifications; depth of experience; performance; and other business and organizational needs. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical / dental / vision insurance and 401(k) retirement plans, subject to applicable plan terms, and / or eligibility for other compensation.
