Job descriptionLead RN / LPN (AM shift) : Full-time, Monday–Friday, typically 7 : 00 AM–3 : 00 PM, with flexibility based on facility needs and participation in a licensed staff weekend on-call rotation (including 1–2 holidays per year). RN / LPN – PM & NOC : Full-time and part-time opportunities on PM shift; part-time opportunity on NOC shift. 16 / 24 Status RN / LPN : Work only 3 weekends per month with an enhanced rate of pay – ideal for those with busy weekdays. Competitive hourly wages based on experience and role (RN / LPN). Committed Hours Pay differential for 72–80 hours per pay period (RN : +$12 / hr; LPN : +$10 / hr). Sign-on bonuses (contingent on committed hours) : RN : $5,000 with 64 committed hours per pay period. LPN : $4,000 with 64 committed hours per pay period. Comprehensive benefits package, including : Paid Holidays, Vacation, Sick Pay, Bereavement Pay. Health, Dental, Vision, Accident, and Short-Term Disability. 401(k) retirement plan with company match. Employer-paid Group Life insurance. Provide daily leadership and support to frontline staff, ensuring safe, high-quality resident care. Coordinate day-to-day clinical operations including resident care, discharge planning, care supervision, and infection monitoring. Teach and support staff, including providing CBRF course instruction. Participate in weekend on-call rotation and occasional holiday on-call coverage. Provide direct supervision to LPNs, CNAs, and other non-licensed personnel on the unit. Complete comprehensive assessments, identify changes in condition, and implement appropriate interventions. Participate actively in the care planning process and ensure care plans are consistently followed. Administer medications and treatments in accordance with physician orders and facility policies. Maintain high standards of professional documentation and uphold all pharmaceutical and nursing policies. Communicate clearly and respectfully with residents, families, and the interdisciplinary team while promoting dignity, safety, and comfort. Foster a positive, collaborative team environment and champion Shady Lane’s commitment to quality and compassion. Provide direct supervision to CNAs and other non-licensed personnel assigned to the unit. Observe residents closely, recognize changes in condition, and implement appropriate interventions or report promptly to the RN / physician. Assist with development and communication of resident care plans and ensure that care is delivered in a dignified, respectful manner. Administer medications and treatments according to orders and facility policies. Maintain accurate, timely documentation that meets professional and regulatory standards. Support a culture of teamwork, respect, and excellent customer service for residents and families. Current, active RN or LPN license in the State of Wisconsin. Passion for serving older adults in a compassionate, dignified, and resident-centered way. Strong teamwork, communication, and problem-solving skills. Ability to work effectively in a fast-paced environment while maintaining high standards of care and professionalism.
