Schedule : Day Shifts, 8-Hours

Position Summary :

To provide quality clinical and psychotherapeutic services to the youth and adult patient population and their families; to serve as a member of the interdisciplinary team supporting the organization’s treatment program and philosophy and assure the deliverance of quality treatment to patients and their families.  Oversees all aspects of clinical care and treatment delivered to patients assigned to his / her caseload. Works collaboratively with other departments within the facility  to ensure the safety and well-being of  patients and  to develop clinically sound, therapeutic interventions and treatment goals for patients as clinically indicated; conducts ongoing assessments and monitors progress of patients assigned to their caseload; Assists in the development of clinical programs and therapeutic interventions for patient; complies with all regulatory and facility policies, procedures and protocols, monitors patient safety and patient treatment; fosters a positive, therapeutic milieu for patients and demonstrates respect for co-workers, patients, family members and outside agency representatives. Monitors customer satisfaction with clinical services and serves as liaison with agency or referral sources to resolve problems or issues; follows all HR policies / procedures; Reports all serious occurrences and incidents and keeps agencies, internal managers, family members informed of issues / problems involving their patients.

Essential Functions :

Participate in interdisciplinary treatment team meetings and patient staffing. Provide useful information, which can be utilized in the care of the patient. Maintains goals & modifications daily

Meets with treatment team weekly to provide input into patient’s care and progress. Works closely with the assigned case manager to coordinate services and care including treatment interventions, communication to family members and outside agency reps., arranging services & coordinating discharge.

Participates in activities which enhance professional growth and development.

Attends all mandatory training as assigned. Maintains up to date documentation of certifications and licenses to human resources.

Independently seeks out training and development opportunities. Makes effective recommendations for organization training activities. Stays abreast of current issues.

Demonstrates conduct in keeping with ethical standards. Maintains proper boundaries when interacting with patients and their families and co-workers. Maintains confidentiality of former and current patient information.

Respects and safeguards the confidentiality and privacy of MBHS and co-worker information. Serves as a role model for other employees by virtue of exemplary behavior on and off the job.

Complies with facility safety, infection control and security program. Keeps workspace clean and free of work hazards that could pose a safety threat to patients. Complies with company’s policy regarding identification badge. Identifies potential safety & security problems and reports to supervisor and / or safety officer.

Ensures that the clinical program and treatment rendered to patients by staff is safe, clinically appropriate and that patients receive quality care in all areas.

Ensures that patients are provided with appropriate and varied program activities, relevant therapeutic interventions, and that their medical needs are addressed by the nursing staff.

Demonstrates sound judgment, and is able to resolve problems and issues that may arise expeditiously.

Ensures that clinical and operational systems within the facility related to patient care are adhered to and updated as needed.

Ensures that all clinical programming, staffing ratios, and documentation are conducted in accordance with all relevant regulatory agencies.

Keeps Dept. Managers at all levels informed of all serious incidents, allegations, or serious issues related to program operations, safety, or personnel matters. Attends all departmental meetings consistently and timely.

Completes initial psychosocial assessments within the first 72 hours in a comprehensive and accurate manner.

Analyzes the information collected in the psychosocial assessment as well as reviewing information collected by other departments for initial admission assessments & develops the initial ISP / Treatment Plan (Discharge Plan) within the first 3 days. Completes admission assessments and scales and in conjunction with other interdisciplinary treatment team members develops the patient’s ISP / Treatment (Discharge Plan.) Presents the ISP / Treatment (Discharge Plan) goals and discusses the goals with the patient, family members and agency representatives and ensures their understanding of the plan. Obtains signatures and provides copies of ISP / Treatment (Discharge Plan) to patient, family members and agency representatives.

Minimum Qualifications :

Education : Master’s Degree in Social Work or equivalent master’s degree in a recognized mental health field preferred.

Experience :  Minimum of two (2) years of clinical work experience or psychiatric settings

Licensure / Certification :   Professional clinical license issued by the appropriate licensing body in Louisiana (LMSW, LCSW, PLPC, LPC); specialty certification is preferred; American Heart Association BLS (AHA BLS) and Crisis Prevention Institute (CPI) certification. Successful candidates who do not have current AHA BLS or CPI certification will be required to attend company-provided classes during the week of orientation.

Other :   Written and oral communication skills; skills in case management, time management, and crisis intervention.  Knowledge of developmental psychology of youth and adults; needs of youth and adults; Skills in maintaining information as highly confidential; must demonstrate the ability to treat other age populations as needed or have the capacity to develop skills through cross-training and orientation.  Ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation.   Ability to complete required documentation in a timely, accurate manner that is clinically appropriate and within expected timeframes.

Competency :  Position and Age-Related

HIPAA :   Has access to and adds information to the Medical Record.  May only access medical record of assigned patients.

Work Demands / Environment :

Ability to stoop, lift, finger – for example pick, pinch, or type and grasp, ability to clearly and concisely express and exchange ideas via spoken word conveying detailed information to others accurately and on occasion quickly.  Ability to perceive the nature of sounds with no less than a 40 db loss @ Hz, 1000 Hz and 2000 Hz with or without correction.  Ability to receive detailed information through oral communication, and to make fine discriminations in sound.  Exerting up to 10 to 1 pounds of force occasionally and / or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.  Ability to inspect and analyze figures, accounting items, written materials, newsprint, computer terminal characters, transcription and other similar objects at distances generally close to the eyes.  This is a minimum standard based on the criteria of accuracy and neatness of work.  The worker is subject to inside environmental conditions :   Protection from weather conditions but not necessarily from temperature changes.  Although inside work is the rule, it is required that the individual be able to travel.
