What You'll Do :

• Provide compassionate hospice care in patients' homes and in facilities within Kent & Ionia counties
• Participate in a fair, rotating on-call schedule with extra pay
• Coordinate care plans with an interdisciplinary team
• Support patients and families with education and emotional care
