Position Overview : The Long-Term Service Support Coordinator with be working with a multitude of diverse partnering agencies across both health and social service disciplines. This is dedicated to promoting consumer choice and self-management as well as greater community inclusion, access, integration, mobility, and robust livability.

Position Responsibilities : Responsible for providing comprehensive and enhanced person-driven supports to enrollees, ages 3 to 64, that are participating in the Commonwealth's Long-Term Services and Supports Community Partnership program. You will also facilitate enrollee use of assistive technology, sustain enrollee engagement and encourage problem solving, manage date and supporting outcomes evaluation. Please note : home visits are required.

Hours 40 hours per week, Monday through Friday
