Occupational Therapist
We are seeking a highly skilled and compassionate Occupational Therapist (OT) to join our team. In this role, you will be responsible for evaluating and treating patients, developing comprehensive treatment plans, and collaborating with a multidisciplinary healthcare team to promote patient independence and well-being.
Key Responsibilities
Patient Evaluation & Treatment :
- Evaluate new patients within 24 hours or one business day of physician referral . Document reasons for any delays in the patient's medical record.
- Develop and implement effective, individualized treatment plans , securing approval from referring physicians.
- Deliver patient care consistent with the physician's treatment plan.
- Record comprehensive documentation, including evaluations, daily treatment notes, weekly progress notes, and recertification documentation, adhering to the Occupational Therapy Board State Practice Act and all governmental and third-party payer requirements.
- Implement and record all treatment changes according to policy and procedures.
Team Collaboration & Supervision :
Communicate effectively with supervisors and other healthcare team members regarding patient progress, problems, and care plans.Actively participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.Provide direct supervision to Certified Occupational Therapy Assistants (COTAs) , aides, and students, ensuring compliance with state practice acts.Assist the nursing department with training for Restorative Aides.Contribute to in-service training programs for facility staff.Discharge Planning & Equipment Management :
Actively participate in discharge planning for patients.Educate patients' families and nursing staff on maintenance programs and provide caregiver training to prepare for discharge from therapy services.Recommend and facilitate the ordering of essential durable medical equipment (e.g., braces, walkers, canes, elevated toilet seats, grab bars) to enhance patient independence in mobility.Report any issues with departmental equipment promptly to ensure proper maintenance and functionality.Knowledge, Skills, and Abilities
Computer Proficiency : Proficient in data input and utilization of electronic health record systems such as Casamba and Point Click Care .Communication & Comprehension :
Ability to read and interpret technical procedures, as well as policy and procedure manuals.Strong ability to effectively present information and respond to questions from managers, coworkers, and patient families.Problem-Solving & Analytical Skills :
Aptitude for applying mathematical concepts (fractions, percentages, ratios, proportions) to practical situations.Skilled in solving practical problems and interpreting diverse instructions (written, oral, diagrammatic, or scheduled).Education & Experience
Required : Bachelor's degree in Occupational Therapy.Preferred : Graduates of Master's or Doctorate Programs in Occupational Therapy.Licensure : Must hold an active and unencumbered Occupational Therapist license in the state and adhere to all state practice continuing education requirements.
