LCSW Care Manager within our Psychiatric Department at Providence Little Company of Mary Medical Center in San Pedro, CA. This position is Full-Time and will work 8-hour Evening shifts (3pm - 11 : 30pm) to include weekend rotations.

Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management. Recognizing the patient\'s right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).

Responsibilities

Typical responsibilities include collaborating with a multidisciplinary team to coordinate patient care within the Department of Psychiatry, facilitating communication across care teams, and supporting patients, families, and caregivers through care transitions. The role emphasizes recognizing patient self-determination, addressing social determinants of health, and navigating barriers to coping, mental health needs, substance use, and resource limitations.

Qualifications

Required Qualifications :

• Master\'s Degree in Social Work.
• Upon hire : California Clinical Social Worker License
• Coursework / Training : All LCSWs must follow the guideline by Los Angeles County Department of Mental Health for LPS designation and keep their designation in current status.
• 2 years of Healthcare related setting with psychiatric mental health patient population in area of service, preferably in an inpatient setting.
• Within 18 months of hire : Designated as Behavioral Health Clinician to perform 5150 evaluations
• Within 1 year of hire : CPI certification (nonviolent crisis intervention training)
• Computer experience.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby / on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
