FlexStaff is seeking a Regional Home Care Nurse for our client, a non-profit healthcare organization providing home and community-based healthcare and services for the elderly.

PURPOSE :

The Regional Home Care Nurse provides compassionate, skilled nursing care to patients in their homes, working with referrals from partner organizations and internal programs. This role involves regular home visits to deliver services such as wound care, medication administration, and health assessments, as needed. Excellent clinical judgment, independent work ethic, and a patient-centered approach are essential.

RESPONSIBILITIES :

• Conduct regular home visits to provide clinical services, including wound care, medication management, and health assessments.
• Deliver nursing care based on individualized care plans, physician orders, and best practices.
• Monitor patient conditions, assess changes in health status, and communicate updates to the care team.
• Collaborate with partner organizations, care coordinators, and internal teams to address patient needs.
• Educate patients and families on condition management, medication adherence, wound care, and home safety.
• Maintain accurate and timely documentation in the electronic health record system.
• Follow clinical protocols, regulatory standards, and infection control guidelines.
• Participate in case conferences and clinical meetings.
• Travel to patient homes within the service area.
• Operate within scope of practice and meet established competencies.
• Serve as an active Interdisciplinary Team (IDT) member.
