About the Role

As a valued member of our team, you will engage with patients in their homes or Skilled Nursing Facilities, conducting assessments that can last up to one hour. This role allows you to :

• Perform thorough adult and geriatric assessments, including medical history review, diagnosis and treatment planning, health education, and care coordination.
• Administer point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, and families to provide comprehensive care.
• Work closely with Primary Care Physicians on patient education and follow-up care.
• Document assessments and care plans according to Matrix guidelines.
