Summary :
Provide outreach, case management, and support to individuals and families experiencing homelessness. Connect clients to housing, healthcare, benefits, and community resources to promote safety, stability, and independence.
Schedule & Assignment Details :
Full-time : 40 hrs. / week with various shifts (8am–4pm, 4pm–12am, 12am–8am)
Part-time : 14–24 hrs. / week
On-call positions are available
Key Responsibilities :
Conduct street and shelter outreach to identify and engage clients.
Assess needs and develop individualized service plans.
Connect clients to housing programs, mental health care, substance-use treatment, and other services.
Provide crisis intervention and safety planning.
Maintain accurate case notes and documentation.
Collaborate with community agencies and service providers.
Qualifications :
Degree in Social Work or related field, or equivalent experience.
Experience working with vulnerable or homeless populations preferred.
Strong communication, problem-solving, and crisis-management skills.
Knowledge of local social service resources.
