Responsibilities :

  • Oversee and coordinate tailored care plans, ensuring that our clients receive appropriate and timely services.
  • Serve as a strong advocate for clients, ensuring their needs are met in home, medical and care facility settings, including while communicating with healthcare providers.
  • Work closely with team members to address clients needs, and contribute your expertise to the team, helping to enhance the collective knowledge and skills.
  • Participate in after-hours or weekend on-call rotations as needed to provide continuous support to clients.
  • Update family members and other members of the care team and maintain thorough and accurate documentation. Approximately 90% of the role involves field work, with 10% dedicated to follow-up and office-based tasks.
  • Requirements :

  • A masters degree in a related field, or a bachelors degree with at least 2 years of relevant experience in gerontology, mental health, home care, or similar.
  • Available 9am to 6pm Monday-Friday and some weekends.
  • Dependable, adaptable, and accountable, with a strong commitment to providing exceptional care and support to clients.
  • Must be able to lift and maneuver up to 50 pounds.
  • Must be comfortable and willing to drive regularly around the Virginia and DC Metro area.
