Advanced Practice Provider

Overview

Catholic Health is seeking a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join the cardiology team of Dr. Ralph F. Caselnova, a highly respected cardiologist with over 20 years of experience serving the Babylon and greater Long Island community. This is an opportunity to work alongside a board-certified cardiologist with expertise in general, interventional, peripheral endovascular, and non-invasive cardiology. The APP will provide comprehensive care to patients with a variety of cardiovascular conditions, including coronary artery disease, heart failure, hypertension, and cardiac arrhythmias, in outpatient and inpatient settings.

Key Responsibilities

• Conduct detailed cardiovascular assessments, histories, and physical examinations
• Order, interpret, and follow up on diagnostic tests such as EKGs, echocardiograms, stress tests, and lab work
• Manage chronic cardiac conditions and adjust treatment plans under physician supervision
• Perform routine follow-up care and pre / post-procedure evaluations
• Coordinate care transitions between hospital, outpatient, and specialty services
• Collaborate with Dr. Caselnova and the broader cardiology team to ensure high-quality, evidence-based care
• Educate patients and families on disease management, medications, and lifestyle modifications
• Participate in hospital rounds as needed at affiliated hospitals (Good Samaritan University Hospital and St. Catherine of Siena Hospital)
• Maintain accurate and timely documentation in the electronic medical record (EMR)
