Hospice LPN Per Diem Weekend

Eden Hospice is hiring a Licensed Practical Nurse to provide in-home services to our patients throughout Washoe and surrounding counties.

Job Summary :

• Provides skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseload.
• Review caseload with supervising RN on a regular basis as established by best practice and / or State guidelines.
• Obtains necessary physician orders and collaborates with the physician as the patient's needs dictate.
• Assess and provide patient and family / caregiver education and information pertinent to diagnosis and plan of care.
• Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members (e.g. hospice aide, RN, OT, PT, MSW, ST, Dietitian, Chaplain or Hospice Manager)
• Uses equipment and supplies effectively and efficiently.
• Provides appropriate pain / symptom management. Evaluates patient's response to treatments / medications.
• Actively participates in Quality management program.
• Completes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.

Education / Experience Requirements :

One year experience in an acute care setting within the last 24 months;
