Dynamic is seeking professional, reliable, and caring Home Health Vocational Nurse (LVN) to join our Medicare-certified Home Health Care Team! We have an Immediate opening for a PER-DIEM LVN Home Health Nurse to provide Home Health visits to our patients across the Greater Los Angeles and Ventura Areas.

Responsibilities :

• Implement plan of care initiated by the Registered Nurse
• Provide accurate and timely documentation, consistent with the plan of care
• Assess and provide patient and family / caregiver education, and information pertinent to diagnosis and plan of care
• Use equipment and supplies effectively and efficiently
• Be responsible for following all company policies and procedures
• Perform other duties as assigned
