An opportunity is available for an experienced and compassionate Registered Nurse to provide skilled home health care in OFallon, MO. This contract position offers the chance to deliver patient-centered nursing care in patients homes, ensuring continuity and quality while working independently within a structured and supportive framework.

Responsibilities include conducting thorough OASIS assessments, developing and managing individualized care plans, and providing skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management. The role involves daily travel within an assigned territory, collaboration with interdisciplinary teams, and educating patients and families on health conditions and treatment plans. Maintaining accurate electronic documentation using Home Care Home Base (HCHB) and adhering to regulatory standards are essential components of this position.

Key Qualifications :

• Active RN license in good standing for the state of practice
• Minimum one year of recent home health nursing experience, with field experience preferred
• Proficiency in Home Care Home Base (HCHB) and OASIS assessments required
• Strong clinical assessment, critical thinking, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to manage a flexible field-based schedule and work independently
• Bachelor of Science in Nursing (BSN) preferred
• Knowledge of Medicare and home health compliance standards is a plus
