Our behavioral health team is expanding psychiatric care services throughout Kings County and we are seeking for compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing team. You’ll play a key role in providing individualized, high-quality mental health care to older adults living in skilled nursing and rehabilitation facilities. This position is ideal for a clinician who thrives in a collaborative environment and is dedicated to improving the emotional well-being of senior patients.
Requirements
Primary Responsibilities
Conduct comprehensive psychiatric assessments and routine follow-up visits for residents in post-acute and long-term care settings.
Develop, implement, and adjust evidence-based treatment plans for patients managing both chronic and acute psychiatric conditions.
Prescribe and monitor psychotropic medications while evaluating patient progress and treatment outcomes.
Collaborate with an interdisciplinary care team—including physicians, nursing staff, and social workers—to ensure coordinated and holistic care delivery.
Provide supportive counseling, therapeutic guidance, and mental health education to residents and their families.
Maintain precise and compliant documentation within the organization’s electronic health record (EHR) system.
Active New York State Nurse Practitioner license and current Psychiatric–Mental Health Nurse Practitioner (PMHNP) certification.
Ability to work independently with confidence while fostering strong collaboration across care teams.
Compassionate, adaptable, and committed to providing exceptional patient-centered psychiatric care.
Excellent interpersonal, diagnostic, and communication skills—especially with geriatric and long-term care populations.
Proficient in EHR documentation and comfortable utilizing telehealth technology when needed.

Base salary : $150,000–$175,000 annually (based on experience).
