C4 was founded in 1972 to help people released from psychiatric hospitals into the Uptown and Edgewater communities stabilize in the community. Since that time, C4's service area has expanded to rekindle hope for individuals and families on Chicago's north and west sides, remaining committed to community-level, system-focused interventions; C4 always seeks to avoid restrictive levels of care and maximize families' agency to lead their recovery, allowing them to heal in their homes and community. C4 is the leader in building hope and breaking down barriers - from the personal to the systemic - for Chicago's systematically disadvantaged communities through the delivery of effective, empirically supported, and trauma-informed mental health services. C4 seeks to equip 7,000 individuals annually to help them move from trauma to living, to working and thriving within their community.

Purpose :
The Program Assistantwill assist with office-based operations, participate in daily team meeting, serve as liaison between clients and staff, such as supporting office walk-ins and calls from clients / family supports, and directly support the ACT team by coordinating and monitoring team schedules. As part of the team, he or she shall assist the Supervisor in providing the quality assurance and oversight necessary to ensure adherence to the ACT fidelity model, as well as direct ACT services with productivity expectations. Minimum Qualifications :
- BA / BS required, preferably in Social Work, Psychology or Counseling
- Bilingual (Spanish) preferred
- 2 years minimum clinical experience working with adult clients with serious mental illness and substance use needs.
- A well-maintained personal vehicle for work related use, valid Illinois driver's license, good driving record, and active vehicle insurance are required

Works closely with the ACT Program ManagerLeads ACT staff meetings and treatment planning meetings with ACT staff in a manner reflecting compassion and a clear understanding of clients' unique circumstances and individual recovery needsDirects the client admission process, treatment, rehabilitation and support services of the program in coordination with the ACT Program Manager and psychiatristTrains and supervises employees to procedures and clinical functions identifying and respecting each person's preferred learning style.Works with ACT Program Manager to assist in the creation and updating of the ACT Procedural ManualSchedules employees to provide adequate coverageCreate a welcoming and inclusive environment where families feel valued and respected.Assist with the client admission process, treatment, rehabilitation and support services of the program in coordination with the psychiatristEnsures compliance with all applicable laws and accreditation requirements related to service delivery policies, practices, and reporting standardsCompiles reports and gathers data as requested by ACT Program ManagerAdheres to confidentiality practices and procedures according to agency policies.Maintains compliance, at all times, with agency policies and procedures.Adheres to ethical practicesOther duties as assigned by supervisor
