About the Role :
We are seeking a compassionate, skilled, and dual-certified Family and Psychiatric Mental Health Nurse Practitioner (FNP / PMHNP) to join our growing telehealth team. This role focuses on providing comprehensive, integrated care to patients with Substance Use Disorders (SUDs) and co-occurring mental and physical health conditions through secure virtual platforms.

Key Responsibilities :
- Conduct comprehensive psychiatric and medical evaluations via telehealth for patients with SUD and co-occurring disorders.
- Develop and manage individualized treatment plans that may include medication-assisted treatment (MAT), mental health support, and primary care interventions.
- Prescribe and monitor psychotropic and primary care medications in accordance with state and federal regulations.
- Provide ongoing follow-up visits to assess progress, adjust medications, and offer counseling and education.
- Collaborate with an interdisciplinary team including therapists, case managers, and addiction specialists to ensure holistic, integrated care.
- Maintain accurate and timely documentation within the electronic health record (EHR).
- Participate in case reviews, team meetings, and continuing education to maintain clinical excellence.
- Promote harm reduction, wellness, and recovery-based approaches in all patient interactions.
Qualifications :
Current certification as both FNP and PMHNP (dual-certified required).Active, unrestricted NP license in the state(s) of practice.DEA license with authorization to prescribe controlled substances (including buprenorphine waiver preferred or willingness to obtain).Minimum 2 years of clinical experience as an NP, preferably in addiction medicine, psychiatry, or integrated behavioral health.Strong knowledge of MAT protocols (Suboxone, Vivitrol, etc.) and evidence-based addiction treatment.Excellent communication skills and comfort using telehealth platforms.Ability to work independently while collaborating with a remote multidisciplinary team.Preferred Experience :
Experience working with underserved populations or community health settings.Familiarity with trauma-informed care and motivational interviewing.Knowledge of co-occurring chronic medical conditions common in SUD populations.Compensation and Benefits :
