Shift : MondayFriday, 85, with potential weekend rotation and on call

Job Description :

Sunbelt Staffing is seeking an experienced Home Health RN for a contract role in Ronceverte, WV. This position involves field-based patient care, completing accurate OASIS assessments, and managing a caseload with professionalism and autonomy. Candidates should be confident working independently and comfortable navigating rural territory.

Key Responsibilities :

Conduct patient admissions, routine visits, and discharges

Complete OASIS documentation using Homecare Homebase (HCHB)

Provide skilled nursing services including wound care, IV therapy, and medication management

Educate patients and caregivers on treatment plans and chronic disease management

Communicate with physicians and interdisciplinary care teams

Qualifications :

Active West Virginia RN license

Minimum 2 years of nursing experience, including at least 1 year in home health

Proficiency with OASIS and Homecare Homebase (HCHB) required

Current BLS certification

Valid drivers license, dependable vehicle, and auto insurance

Ability to meet all Joint Commission compliance standards
